In an effort to get you on the track quicker when you visit us at MVP, we have included all of the required forms and paperwork here on our web site for your convenience.   These are required by our insurance carrier for all riders visiting Mountain View Park, regardless of age or experience.  The appropriate paperwork must be filled out by every person riding the tracks at our facility the first time you visit MVP.   After that, they will be kept on file at the track for the entire season so you don't have to do the paperwork every time you visit MVP.


IMPORTANT NOTE (NO EXCEPTIONS):  As part of our insurance requirements, if you are bringing a minor child (under 18 years of age) to the track to ride and you are NOT that child's parent or legal guardian, you MUST have that child's parent's or legal guardian's APPROVAL AND NOTARIZED SIGNATURE on our Guardian Release form (linked below).  Without this SIGNED AND NOTARIZED Guardian Release form, that minor child will NOT be allowed to participate at this facility.  We have a notary on site for your convienience.

This is the same form that we use at the track to sign up riders that have never riden at our track before. You MUST have one of these forms on file with MVP in order to ride at our facility. If you chose to print this form and fill it out in advance of coming to our facility, it MUST be signed and notarized. Otherwise, this form is always available at the registration desk and we have a notary on site to notarize the forms after you have signed them in our presence.

ALL riders under the age of 18 must have a completed minor release form (in addition to the Participant Form shown above) to be allowed to ride at our facility. Note: ONLY ONE rider per form, but you can sign up a single rider for multiple classes on the same sheet. Otherwise, this form is always available at the registration deskthe morning of the race.
